For one suspended moment, nobody spoke.
Derek remained beside the door with his arms folded. Jessica stood slightly behind him, dressed as if she were meeting someone for lunch rather than intruding on another woman's medical appointment. Her perfume reached Sarah before either of them crossed the room.
Dr. Evans lowered the ultrasound probe.
"Who are you?"
"I'm her husband," Derek said.
The doctor looked toward Sarah.
"Did you invite him?"
"No."
Jessica shifted.
Derek's expression tightened.
"I have a right to know whether the child she's carrying is mine."
Dr. Evans's face became unreadable.
"Your marital status does not give you permission to enter a patient's examination room."
"I came because Sarah asked me to attend."
"I told you when the appointment was," Sarah said. "I didn't invite Jessica."
Jessica lifted her chin.
"I'm here to support Derek."
Sarah almost laughed.
"Of course."
Dr. Evans reached toward the call button.
"Mrs. Collins, would you like them removed?"
Every sensible part of Sarah wanted to say yes.
She wanted security.
She wanted the door shut.
She wanted Derek and Jessica removed from the one place where she should not have had to defend herself.
Then she remembered the divorce agreement.
Margaret's trash bags.
Jessica's message.
Derek's absolute certainty.
Sarah looked at the monitor.
"No."
Dr. Evans paused.
Sarah met Derek's eyes.
"Let him stay."
A trace of satisfaction crossed his face.
Jessica relaxed.
They believed Sarah had surrendered.
Dr. Evans repositioned the probe.
"Very well. But this remains my patient's appointment. If either of you interferes, you will leave."
Derek nodded impatiently.
"How far along is she?"
The doctor looked at Sarah first.
Sarah gave permission with a small nod.
"Measurements indicate approximately eleven and a half weeks."
Derek's face changed.
Not dramatically.
His arms simply loosened.
"That's wrong."
Dr. Evans glanced at him.
"The measurements are internally consistent."
"The surgery was eight weeks ago."
Sarah watched him calculate.
The Asheville weekend.
The nights before his procedure.
The weeks he had apparently erased from his own argument.
Dr. Evans continued calmly.
"Based on the fetal measurements, Mrs. Collins was already pregnant before the date you're describing."
Jessica looked at Derek.
He stepped toward the monitor.
"Ultrasounds can be wrong."
"Dating has a margin of error," Dr. Evans said. "It does not have the kind of margin you're asking for."
"So she conceived before my vasectomy."
"That is what the measurements indicate."
Sarah waited.
She did not know what she expected.
An apology, perhaps.
Not a grand one.
Just some visible recognition of what he had done.
Derek looked at her.
"That still doesn't prove I'm the father."
Whatever remained of Sarah's hope for him went quiet.
She stared at him.
"You really need me to be guilty."
"I need proof."
"No. You needed a reason to leave."
Jessica folded her arms.
Derek ignored her.
"If the pregnancy happened before the surgery, you still could have been with somebody else."
"I wasn't."
"Then a DNA test shouldn't bother you."
"It doesn't."
He hesitated.
Sarah sat slightly higher.
"I'll take one."
Jessica's eyes flicked toward Derek.
Sarah continued.
"I want the result documented."
"Good."
"And when it says what I already know, you don't get to pretend you never said these things."
Derek looked away.
Dr. Evans interrupted.
"We can discuss appropriate testing options separately. Right now there is something more immediate."
Sarah's attention snapped back to the monitor.
"What?"
Dr. Evans moved the probe.
A small shape appeared.
Sarah saw movement.
Then the doctor turned up the sound.
A rapid heartbeat filled the room.
Sarah covered her mouth.
She had imagined this sound for years.
It was faster and more delicate than she expected.
Her eyes blurred.
"That's my baby?"
"That's one of them."
Sarah's hand stopped halfway to her face.
"What did you say?"
Dr. Evans shifted the probe.
Another shape appeared.

Derek grabbed the back of a chair.
Jessica stepped away from him.
Dr. Evans smiled at Sarah.
"You're carrying twins."
Sarah could not speak.

"Are they okay?"
"Both heart rates look strong. Both are measuring consistently."
Sarah reached toward the screen.

"My babies."
Beside her, Derek whispered, "Twins?"

He looked almost offended by the image.
As if the monitor had betrayed him.
Jessica had gone pale.
Sarah noticed.
"You knew we tried for years," Sarah said to her.
Jessica's mouth tightened.
"This isn't about me."
"You made it about you when you walked into my ultrasound."
Derek interrupted.
"What does the vasectomy have to do with this?"
Dr. Evans looked at him.
"What exactly were you told after the procedure?"
"That it worked."
"Did you complete a post-procedure semen analysis?"
Derek did not answer.
Sarah turned toward him.
"Derek?"
He rubbed his jaw.
"I didn't have time."
Sarah thought she had misheard.
"What?"
"I was busy."
"You told me the doctor confirmed you were sterile."
"I said the procedure had worked."
"You told me you had been cleared."
"That's what usually happens."
Dr. Evans's expression sharpened.
"Patients are generally instructed not to assume sterility until follow-up testing confirms it."
Jessica stared at Derek.
"You told me there was no possibility."
"Stay out of it."
Her eyes widened.
Sarah saw something pass between them.
A fracture.
Small, but unmistakable.
"So you never confirmed anything," Sarah said.
Derek turned on her.
"That doesn't change what you did."
"What I did?"
"You got pregnant."
"With your children."
"We don't know that."
Sarah laughed softly.
The sound surprised her.
Not because anything was funny.
Because the alternative was screaming.
Dr. Evans removed the probe.
"Mrs. Collins's blood pressure is elevated. I need to complete this examination without further conflict."
Derek pointed toward Sarah.
"She's manipulating this."
Dr. Evans stood.
"I have watched you enter an appointment without permission, accuse a pregnant patient of infidelity despite medical evidence contradicting your timeline, and argue with her after learning she is carrying twins."
Derek's jaw tightened.
"I'm trying to establish facts."
"Then begin by listening to them."
Sarah looked down so he would not see the sudden relief on her face.
It had been more than a week since anyone had said no to Derek.
The doctor turned back to Sarah.
"A twin pregnancy requires closer observation. I want to monitor your blood pressure and schedule additional scans."
"Are they in danger?"
"At the moment I see two viable pregnancies. I want to keep it that way."
Derek gave a humorless laugh.
"Stress. Of course."
Sarah looked at him.
"You walked out on me the morning I told you."
"Because you lied."
"The scan just proved your timeline was wrong."
"It proved the pregnancy is older."
"Listen to yourself."
"I want the DNA test."
Sarah wiped the gel from her abdomen and pulled her dress down.
"Fine."
Everyone turned toward her.
"I'll do it."
Dr. Evans explained that noninvasive prenatal paternity testing could be considered and that Sarah's consent was essential.
"I consent."
Derek's confidence returned slightly.
Sarah looked directly at him.
"Not because I owe you proof. Because I want a permanent record of what happened here."
Jessica scoffed.
"We'll see what it says."
Sarah turned.
"Yes."
Her voice was quiet.
"We will."
Dr. Evans asked Derek and Jessica to wait outside while she finished the appointment.
Derek objected.
Dr. Evans called security.
A guard appeared within minutes.
Jessica left first.
Derek lingered.
"I'll be outside."
Sarah did not answer.
The door closed.

She bent forward and cried.

"I thought he'd be happy," Sarah managed.

"How long?"

"And then he chose the vasectomy?"
Sarah nodded.
"He said trying was ruining us."
"Was it a mutual decision?"
"No."
Sarah looked at the ultrasound pictures.
"He said another child wasn't practical."
Dr. Evans frowned.
"Another?"
"Derek talks about children as expenses sometimes. We don't have any."
Sarah studied the two tiny forms.
"I don't know how I'm supposed to do this alone."
"You don't have to solve the next eighteen years today."
"I might lose my house."
That caught the doctor's attention.
Sarah told her about the divorce papers.
The financial demands.

Jessica.
The prepared suitcase.
When she finished, Dr. Evans was silent for a moment.
"Do you have legal representation?"
"No."
"Get some."
Sarah looked up.
"I don't even know where to start."
The doctor took a business card and wrote on the back.

Sarah accepted it.
"Is she good?"

For the first time that day, Sarah almost smiled.

Sarah looked at the card.
